BILLING
Children's Heart Center has staff to deal with your insurance and billing needs. We wish to provide you with as much assistance as
possible in the filing of your insurance forms. Our office bills all insurance
plans for you at no charge. Please be prepared to pay any co-payments, unmet
deductibles, or co-insurance charges as required by your insurance company. If
you have no insurance coverage, we will refer you to the state financial
assistance programs. If you have questions concerning our participation in an
insurance plan, regarding fees, or any unpaid balance, we ask that you contact
our Billing Office at 702.990-4844.

Medical Records
All protected health information (PHI) requests must be in writing. Please use Children’s Heart Center’s Authorization for the Release of Health Information to expedite your request for your medical records. Please forward all medical record requests and a legible copy of your photo identification to:

Please note: Medical Records copied for reasons other than continuity of care are subject to a copy fee (NRS 629.061). These include but are not limited to legal requests, investigative agencies, insurance companies, and patient personal use requests.
